#245 Activity worker should use BulkheadSync for parallel execution

Activity worker processes activities sequentially (1 at a time). Should use BulkheadSync like orchestrator for 10x concurrency per worker. Current: 4 workers x 1 = 4 concurrent. Expected: 4 workers x 10 = 40 concurrent.
