#668 Bulkhead timeout has race condition with abandoned threads

When task times out in bulkhead, the thread may still be running. Fixed by: 1) Adding immediate error update with {abandoned: true} 2) Adding documentation about race condition mitigations 3) Enhanced logging for abandoned task scenarios
